Community Health Delivery Partnership

Our vision

Increased access to equitable, high-quality essential health services through community-based primary health care.

Our mission

The CHDP champions a country-led, data-driven approach to identify policy and program actions to advance national priorities, with the aim of supporting countries to strengthen the health system at the community level, including community health workers’ status, rights and protections.

Why community-based primary health care?

Community-based primary health care provides a sustainable pathway for countries pursuing universal health coverage and the attainment of the Sustainable Development Goals. Extending preventive, promotive, and curative health services into communities is a critical aspect of ensuring equitable access to high-quality primary health care.

Primary health care delivered at the community level offers:

A high return on investment
A pro-equitable approach
A pro-equitable approach
UNI233942
Integrated services that address multiple health needs

This is an especially critical moment to accelerate community-based primary health care efforts:

  • Recent milestones, such as the 45th anniversary of the Alma-Ata Declaration and the 2023 Monrovia Call to Action, emphasize the importance of these initiatives.
  • Key stakeholders, global partnerships and country commitments are aligning around primary health care, offering a unique opportunity to strengthen community-based efforts.
  • By leveraging these commitments and partnerships, countries can make progress towards universal health coverage and the Sustainable Development Goals.
