Technical assistance request form

Greetings and thank you for taking the time to complete this form. The purpose of this form is to help the Community Health Delivery Partnership (CHDP) partners systematically track and consider requests from the Ministries of Health for technical assistance. Please note that completing this form does not indicate or confirm a commitment on behalf of the CHDP. Each request will be shared with CHDP partners and carefully considered alongside available resources.

The purpose of this is to assist countries and ministries of health in accelerating their community health programs whilst reinforcing the objectives of the CHDP to enhance alignment and coordination of partners, particularly as it relates to the coordination of technical assistance to mitigate duplication and fragmentation.

Requests for technical assistance should be focused on resolving a critical bottleneck that is impeding progress for the country’s community health program.

We kindly request that this form be completed by a designated focal point at the Ministry of Health with strong linkages to the community health program and in coordination with community health-focused stakeholders in-country (e.g., MOH-led Technical Working Group for community health).
